Insurance Information.

Here at Alizadeh & Schreiner Orthodontics, we are happy to accept assignment of your insurance benefits; however, it is important to remember that our agreement is with you and not your insurance company. If for some reason your insurance does not pay the amount that was expected, or if you lose your insurance coverage during treatment, the unpaid balance of your estimated insurance benefit will be transferred to your balance and a new contract can be created.

It is very important to notify our office immediately if there are any changes in your insurance coverage. This would include a new insurance carrier, a new job, any loss of coverage or if additional coverage has been obtained.

As a service to you, our office will contact your insurance company to verify any orthodontic benefits you may have. Please remember, this is not a guarantee of benefits, and we cannot guarantee your insurance will pay the amount they provided to our office in that verification. Most insurance companies will not pay our office in full when the claim is submitted or when treatment has started. Each insurance company has their own formula that they use to calculate their payment schedule. Your insurance company will be making either monthly or quarterly payments to our office during the course of treatment.

If we are not able to obtain insurance payment within 45 days after the start of treatment, we will transfer the insurance balance to your contract and have you work with your insurance company. Our office is more than happy to help you with this process and provide you with any paperwork you may need to receive reimbursement. The insurance companies are more willing to work with their member than to work with a healthcare provider.

Transfers or Discontinuation of Treatment.
The terms of the contract are subject to change if you transfer to another orthodontist during treatment. The fee will be prorated according to the amount of treatment that has already been performed. At the time of the transfer, it is possible that monies are owed to Alizadeh & Schreiner Orthodontics or our office may in fact owe you a refund.
